Overview

Oxidative stress — an imbalance between reactive oxygen species (free radicals) and antioxidant defences — is one of the most common convergence mechanisms by which multiple factors on this list ultimately impair fertility. When ROS levels become excessive, they attack sperm membranes, impair motility, and critically, fragment sperm DNA. High sperm DNA fragmentation can cause failed fertilisation, failed implantation, and recurrent miscarriage — and is invisible on a standard semen analysis. It requires a separate, specialised test.

How It Affects Fertility

  • Lipid peroxidation of sperm cell membrane — destroys structural integrity
  • Impaired sperm motility from mitochondrial damage
  • Sperm DNA fragmentation — a hidden cause of failed IVF and recurrent miscarriage
  • Reduced fertilisation and embryo development capacity

Symptoms to Watch For

  • Typically silent — requires specialised DNA fragmentation index testing
  • Associated with failed IVF cycles and recurrent miscarriage despite "normal" semen analysis
  • Normal semen analysis is possible despite critically high oxidative stress

What You Can Do

  1. 1 Request sperm DNA fragmentation (SDF) testing specifically — it is not part of a standard semen analysis
  2. 2 Antioxidant protocol: CoQ10 (200–600mg), vitamin C (1000mg), vitamin E (400IU), selenium (100–200mcg)
  3. 3 Identify and address all upstream causes on this list — they all converge on oxidative stress
  4. 4 Abstain from smoking completely — cigarettes are among the highest single oxidative stress drivers
  5. 5 3 months of targeted antioxidant intervention can meaningfully reduce SDF index
